Health Consumers Tasmania invite you to fill in their fourth survey on the impacts of COVID-19.
The Survey takes less than 10 minutes and is open until Sunday night (19th July).
As the impacts on our community from this virus change it is really important the voices of patients, families and friends, and those at risk are heard.
To assist people who can not access the internet, a print copy is available. Written responses may not be included in any initial reports because of the time taken to receive them, but they do contribute to an understanding of the stories people need to tell.
Alternatively, you can help someone answer the survey by you filling it in for them online as you talk to them. Please note you can take the survey multiple times from one device.
